Patient Safety

   Overview
 Preventable medical errors cause tens of thousands of preventable deaths each year, according to reports from the Institute of Medicine and the Commonwealth Fund (IOM, 1999) (Bleich, 2005). The VNAA Blueprint’s patient safety best practices address some of the most serious risks home health patients face.  For example, proper medication reconciliation can minimize or eliminate medication errors, thereby preventing adverse events or unnecessary hospitalization.  Assessing the risk of falls–and implementing fall prevention interventions–addresses the most common cause of nonfatal injuries and hospital admissions for trauma for seniors. Teaching patients and their families to recognize red flags and to take appropriate action fosters patient engagement and helps avoid hospitalization and other negative outcomes.
