Overview
 

The first few days after a patient is admitted to hospice are critical. It is an emotionally and physically stressful time for both patients and caregivers who often feel overwhelmed and anxious. This module provides concrete and specific recommendations of the steps to take as patients and their caregivers begin care in hospices and palliative care.  Expert implementation of these recommendations of care initiation leads to positive outcomes for the patient, caregiver and agency. This module includes best practices in CRITICAL INTERVENTIONS and CRISIS MANAGEMENT to ensure safe, timely, efficient and effective care delivered to the patient.
