  • Measure competency of staff annually in teach back and motivational interviewing.
  • Measure the number of patients who increase in "readiness" as measured by importance and confidence ruler at 10 days and the number of patients who score greater than 7 at 10 days on the importance and confidence ruler, indicating readiness for change.
  • Measure the percentage of patients who identify at least one self-management goal.
  • HHCAHPS Scores--patient perception.
