As Director of New Programs at Dominican Sisters Family Health Services, Mary Jean McKeveny leads a team of care transitions certified nurses, who provide direct services to patients, such as medication management support, identification of red flags and PCP follow-ups. Focusing on outcome improvement, Mary Jean pioneered the use of Eric Coleman’s Care Transitions Intervention Program at her agency and has achieved remarkable results, especially related to Acute Care Hospitalization. Her efforts developed a framework in the home care workflow to identify patients at high risk for readmission and establish best practice protocols, including root cause analysis of all 30-day readmissions.

 

With her extensive knowledge, McKeveny has developed an orientation program for all new employees and specialized an educational training program for clinicians and nurses based on Coleman’s intervention model. For her remarkable achievements and expertise, Mary Jean became VNAA Program Manager of the Year 2013.
