Measurement and Evaluation: Frontloading
The measurement criteria and tools identified below were evaluated by the VNAA Best Practices Work Group, and downloadable materials are offered for duplication and use. - Measure the percentage of high risk patients that received frontloading during first two weeks, or had 60 percent of visits provided in the first two weeks (include heart failure, chronic obstructive pulmonary disease and those with high risk medications).
- Measure the percentage of patients with telemonitoring in use in the first two weeks of care.
- Measure the percentage of patients hospitalized within the first two weeks with frontloading.
- Measure the percentage of patients hospitalized within the first two weeks with frontloading and telemonitoring in place.
- Measure the percentage of patients who received the first visit in 24 hours and contact (visit or call) within 48 hours of hospital discharge.
